Haemorrhoids
The most common diagnosis treated in proctology day centres and outpatient clinics is haemorrhoids.
Hemorrhoids are divided according to their location into internal and external. The internal golden vein is located inside the rectum and is primarily betrayed by bleeding. External haemorrhoids are painful to the touch, they tend to burst, itch, burn, swell and can even become inflamed.
In general, today we talk about the division into 4 stages of hemorrhoidal disease. The first is manifested by itching, burning, or it is without manifestations, it is detected only on examination. In the second stage, the problem is exacerbated when the individual venous nodes bloat during emptying and prolapse, i.e. dislocation, may occur. However, after a stool, they reponate themselves inside. The third stage of haemorrhoids is even more complicated as spontaneous repositioning no longer occurs. Thus, the patient pushes the golden vein himself, causing swelling or inflammatory complications, which may also manifest as mucus or blood on the underwear. The fourth stage is already characterized by complications – bleeding, inflammation, thrombosis of hemorrhoids protruding from the anus.

Fistula
The term anal fistulas (anal fistulas) can be used to describe tunnels or pathways around the rectum. They are formed if the natural glands of the rectum become infected. Fistulae are divided into complete and incomplete. Complete fistulas mouth up in the rectum, which can cause leakage of stool. In the case of incomplete fistulas, only the pus that has formed as a result of inflammation leaks out. For fistulas, we look for the internal and external orifice and determine the position relative to the animal. Their treatment consists of surgical removal, if indicated by the patient.
Polyps
In the presence of polyps and tumours, we assess the size, shape, surface, location in relation to the animal and take a histological sample in order to exclude malignant tumour /cancer/. Subsequent removal is possible surgically as part of same-day surgery.
Abscesses
Anal abscesses are medically an inflammatory disease with the presence of pus in the rectal area. They can cause fever, uncomfortable pain and make it impossible for patients to do normal activities such as sitting or sleeping. We deal with abscesses at a diagnostic level and can help patients to arrange treatments at specialist centres.
Fissure
An anal fissure is a tear in the area of the end of the anal opening. The exact causes are not known, but it is thought to be the result of too hard stools or prolonged thin stools. Initially, conservative treatment is chosen with the help of ointments or medicines. For further treatment, dilators can be used. In case of indication by the doctor, we proceed to surgical treatment.
Diagnostics
The cornerstone of successful treatment is the accurate diagnosis of the disease, which is based on anamnestic data obtained by interviewing the patient and subsequent examinations according to the type of disease. During the interview, we try to obtain data on bleeding, the nature of the pain, duration of the discomfort, frequency, discharge, presence of itching, habits and lifestyle, diet, etc. Based on these data, we determine a diagnostic test plan.
We usually start with an indagation examination of the rectum, followed by an endoscopic examination with a proctoscope (examination of the anal canal area) or a rectoscope, which examines the rectum in greater depth. These examinations give us a visual picture of the condition of the anal canal and lower rectum, anatomical proportions, morbid findings on the wall and mucosa such as the presence of fissures, fistula holes, haemorrhoidal nodes in various clinical stages, polyps, tumours, inflammatory changes and other findings.
In case of inflammatory-appearing changes on the mucosa, histological samples are taken to exclude malignancy and malignancy, respectively. determining the origin of the inflammation in terms of Crohn’s disease, ulcerative proctitis or other forms of inflammation.
Treatment
Treatment is determined by the stage of the disease based on examinations. Treatment options range from lifestyle modification, diet, physical activity, hygiene habits, conservative treatment with ointments and pills to various types of surgical intervention.
